Power of Choice Survey

Your input is needed on the Power of Choice campaign. The Power of Choice is a project of 360 Youth Services in collaboration with District 203 and District 204 focused on supporting parents and students making healthy choices related to teen alcohol and other substance use. The Power of Choice is asking middle school parents and high school teens to participate in a short survey to provide feedback regarding where they receive information. The deadline to take the survey is Friday, April 30.

District 204 Parent Feedback Survey for the 2021-22 School Year

The district is planning on full in-person learning for the 2021-22 school year. School districts are required by the state to provide the option of remote learning only for students with medical exemptions. According to the Illinois Department of Public Health (IDPH),  “Consistent with the updated guidance from the CDC, families of students who are at increased risk of severe illness (including those with special health care needs) or who live with people at increased risk must be given the option of remote instruction.” Remote learning will only be offered to students who meet the IDPH criteria.

To assist in our planning for next year, we are asking parents/guardians to let us know if you plan to request remote learning if your child meets the IDPH criteria. The number of students who participate in remote learning directly impacts the instructional program the district can offer. Program details and the process for applying to participate will be provided at a later date. At that time, you will be asked to provide a firm commitment for the 2021-22 school year.

Please provide your feedback by Monday, April 26, 2021 at  2021-22 School Year
